F Intravenous administration errors such as incorrect flow rates or failure to monitor a flow rate that results in injury to the client

F Falls that occurs as a result of failure to provide safety to the client

F Failure to use sterile technique when indicated

F Burns sustained by the client as result of failure to monitor bath temperature or equipment

F Failure to monitor a client's condition

F Failure to report changes in the client's condition to the physician
